India’s TB elimination drive: what happened?

India still carries a large share of the world’s TB burden, but the pace of decline has beaten the global average. The new push is less about a new drug and more about finding missing patients and keeping them on treatment through the community.

  • A PIB backgrounder on 13 October 2026 says India’s yearly emerging TB cases fell 21% between 2015 and 2024, against a global decline of 12% (WHO Global Tuberculosis Report 2025); treatment coverage is 92% versus 78% globally.
  • The same day the Ministry launched a JOSH (Joint Squad for Health) social-media campaign on a pilot in 11 States and 55 districts, with MY BHARAT volunteers, NCC cadets and a Class-I officer in each team.
  • Under the intensified TB Mukt Bharat Abhiyaan, screenings have crossed 28.1 crore, over 20 lakh people got TB Preventive Treatment, 5.7 lakh new Nikshay Mitras were added, and 46,118 Gram Panchayats have TB-free certification.
Back Story

What is NTEP?

  • In 2020 the Revised National Tuberculosis Control Program (RNTCP) was renamed the National TB Elimination Programme (NTEP), signalling a goal to eliminate TB before the global 2030 target.
  • The National Strategic Plan to End TB in India 2020-25 (announced June 2020) rests on four pillars: Detect, Treat, Prevent, Build.
  • It aligns with Sustainable Development Goal 3 on health and well-being.
Back Story

What is Pradhan Mantri TB Mukt Bharat Abhiyaan?

  • Launched on 9 September 2022 as a Jan Andolan under NTEP: extra support to patients, more community involvement, and use of CSR.
  • A 100-day drive began on 7 December 2024 in 347 high-priority districts across 33 States and UTs; its methods are now scaled nationwide.
  • Treatment success under the Abhiyan has risen to 90%, above the global average of 88%.

What is the BPaLM regimen?

  • BPaLM combines Bedaquiline, Pretomanid, Linezolid and Moxifloxacin for drug-resistant TB.
  • It shortens treatment to about 6 months. More than 15,000 MDR/RR-TB patients have been started on it under the programme.

What new tools sit behind the screening surge?

Technology and service points cited in the backgrounder
ToolScale or role
Rapid molecular testingMore than 9,800 facilities
Culture and drug-susceptibility labs107 (described as the world’s largest such network)
AI-enabled handheld chest X-raysOver 500 in use; 1,500 being delivered to States and UTs; 34-fold rise in X-ray screening
TB Mukt Bharat AppAI chatbot “Khushi” for symptoms, progress and nearby centres
Ayushman Arogya Mandirs1,87,250 centres (as on 13 Oct 2026) for first-contact screening

How it works

How is the JOSH campaign meant to work?

Youth teams for TB awareness

  1. Pilot in 11 States and 55 districts
  2. Each JOSH team: MY BHARAT volunteers + NCC cadets + Class-I officer
  3. Push screening, early detection, adherence, stigma reduction
  4. Posts and stories amplify the Jan Andolan
  5. Link people to free NTEP diagnosis and treatment
Why examiners care

Why does India’s TB elimination drive matter?

AngleWhat to note
Public healthFaster fall in incidence than the world average; high treatment coverage.
TechnologyHandheld X-rays, AI triage and a chatbot cut the wait for diagnosis.
NutritionCash and food baskets attack the reason patients drop out of long treatment.
CommunityNikshay Mitras and JOSH turn TB control into a people’s campaign.
FederalAyushman Arogya Mandirs and State delivery of handheld X-rays carry the last mile.
Fault Lines

What are the challenges in India’s TB elimination drive?

  • A 21% fall still leaves a large absolute burden; missing asymptomatic cases remain the hard part.
  • Drug-resistant TB needs shorter regimens and tight follow-up; BPaLM coverage is still in the tens of thousands.
  • Stigma keeps people from testing; a social-media campaign helps only if local health workers back it.
  • Private-sector patients must still be notified and supported if national numbers are to stay honest.
Road Ahead

What should be done next on India’s TB elimination drive?

  • Expand JOSH beyond the 55-district pilot with measured screening gains.
  • Keep scaling handheld X-rays and BPaLM with radiographer posts under NHM.
  • Publish district-wise TB-free Gram Panchayat outcomes, not only counts.
  • Hold the link between Nikshay Poshan transfers and treatment completion.
